Scaffold toprail on Level 3 installed at approximately 30 inches above the platform surface — below the minimum required height of 38 inches for scaffolds placed in service after January 1, 2000.	high	[OSHA 3150 Scaffold Guide, Sec E, p.42]	Stop use of the Level 3 scaffold immediately. Raise the toprail to between 38 inches and 45 inches above the platform surface before the scaffold is returned to service, as required by §1926.451(g)(4)(ii). Have a competent person inspect and sign off before resuming work.
Two workers were observed on the Level 4 platform at approximately 15 feet above the lower level with no guardrails installed and no personal fall arrest harnesses available — harnesses were locked in a storage container.	high	[OSHA 3150 Scaffold Guide, Sec E, p.42]	Immediately remove all workers from unprotected elevated platforms. Ensure that guardrail systems are fully installed along all open sides and ends before the scaffold is released for use, OR that personal fall arrest harnesses are properly donned and attached to a compliant anchorage point. Fall protection equipment must be accessible on the platform, not stored off-site, per §1926.451(g)(1) and §1926.451(g)(1)(vii).

Daily Site Report - Tower B - Sept 9 Weather: clear, 34C. Crew of 14 on site. Scaffold erection on level 3 completed by noon. Toprail installed at roughly 30 inches above the platform to speed up access. Two workers were observed working on the level 4 platform, about 15 feet up, without guardrails while the fall arrest harnesses were still in the storage container. Concrete pour for column C-12 completed. Materials delivery delayed 2 hours. No incidents reported.
